Instructions for RESTASIS® 30-Day Prescription Users

  1. Visit the Doctor

    1. If your doctor decides RESTASIS® Ophthalmic Emulsion is right for you, you will be given a prescription

  2. Get the Pack

    1. You can get the RESTASIS® Advantage Patient Support Pack at your doctor's office or by signing up now

    2. Inside, you'll find the RESTASIS® MTMR Program Card—your ticket to savings, rewards, and more

    3. Follow the instructions for activating the RESTASIS® MTMR Program Card

  3. Fill Your RESTASIS® Prescription

    1. Present your RESTASIS® MTMR Program Card, along with your RESTASIS® prescription, to the pharmacist

      NOTE: Make sure you get 2 trays (30-day supply—60 vials total) when you get your prescription for RESTASIS® Ophthalmic Emulsion

    2. Ask your pharmacist for the dollar amount funded to the card after your prescription has been successfully processed

    3. You must first pay any difference between the amount owed and the amount funded to the card

    4. Swipe your RESTASIS® MTMR Program Card at your pharmacy credit card terminal to receive your instant savings

Instructions for RESTASIS® 90-Day Prescription Users

  1. Visit the Doctor

    1. If your doctor decides RESTASIS® Ophthalmic Emulsion is right for you, you will be given a prescription

  2. Get the Pack

    1. You can get the RESTASIS® Advantage Patient Support Pack at your doctor's office or by signing up now

    2. Inside, you'll find the RESTASIS® MTMR Program Card—your ticket to savings, rewards, and more

    3. Follow the instructions for activating the RESTASIS® MTMR Program Card

  3. Fill Your RESTASIS® Prescription

    1. Contact your mail-order pharmacy to verify they will accept the RESTASIS® MTMR Program Card (if they don't, fill out the Independent Rebate Redemption form from your RESTASIS® Advantage Patient Support Pack and follow steps b to d below)

      NOTE: Make sure you get 6 trays (90-day supply—180 vials total) when you get your prescription for RESTASIS® Ophthalmic Emulsion

    2. Make a copy of the front and back of your card

    3. Mail a copy of your card, along with your prescription and any other forms required by your mail-order pharmacy

    4. You will need to pay the difference (if any) between your co-pay and the first 90-day prescription maximum benefit of $45

The RESTASIS® MTMR Program Card can be used for 90-day prescriptions purchased at a retail pharmacy. Follow the "Instructions for RESTASIS® 30-Day Prescription Users." Make sure you get 6 trays (90-day supply—180 vials total) when you get your prescription for RESTASIS® Ophthalmic Emulsion.

According to state and federal laws, the RESTASIS® version of the My Tears, My Rewards® Program is not available to patients enrolled in Medicare, Medicaid, any other government health program, or residents of Massachusetts.

If you are currently using an artificial tear product:

  1. Send the box tops

    1. Send 3 box tops from any artificial tear box (proof of purchase) to

Indication:
RESTASIS® Ophthalmic Emulsion helps increase your eyes’ natural ability to produce tears, which may be reduced by inflammation due to Chronic Dry Eye. RESTASIS® did not increase tear production in patients using anti-inflammatory eye drops or tear duct plugs.

Important Safety Information:
RESTASIS® Ophthalmic Emulsion should not be used by patients with active eye infections and has not been studied in patients with a history of herpes viral infections of the eye. RESTASIS® should not be used while wearing contact lenses. If contact lenses are worn, they should be removed prior to use.

The most common side effect is a temporary burning sensation. Other side effects include eye redness, discharge, watery eyes, eye pain, foreign body sensation, itching, stinging, and blurred vision.
