My Tears, My Rewards® Program Information

Q. How does the RESTASIS® MTMR Program work?

A. Each time you are ready to refill your RESTASIS® prescription, have your RESTASIS® MTMR Card ready and follow the instructions.

Approved Use
RESTASIS® (Cyclosporine Ophthalmic Emulsion) 0.05% helps increase your eyes' natural ability to produce tears, which may be reduced by inflammation due to Chronic Dry Eye. RESTASIS® did not increase tear production in patients using anti-inflammatory eye drops or tear duct plugs.

Important Safety Information
Do not use RESTASIS® Ophthalmic Emulsion if you are allergic to any of the ingredients. To help avoid eye injury and contamination, do not touch the vial tip to your eye or other surfaces.

Instructions for using MTMR Card for 30-day prescription refills and 90-day prescription refills of RESTASIS®

Q. What do I do if my RESTASIS® MTMR Card doesn't work?

A. If you are having a problem with the card, please call 1-855-879-4014 and we will be happy to assist you.
If the pharmacist is having a problem processing the card, they can call Therapy First Plus at 1-800-422-5604.


Q. How can I obtain a replacement RESTASIS® MTMR Card if mine is lost or stolen?

A. Please call customer service at 1-855-879-4014.


Q. Whom may I contact if I have questions about the RESTASIS® MTMR Card?

Q. How can I switch from the MTMR 30-Day Program to the MTMR 90-Day Program?

A. When you fill a 90-day prescription using your RESTASIS® MTMR Card, you're automatically upgraded.

Please note that once you end your participation in one program and begin a new program, you may need to forfeit any accumulated rewards from the previous program.

My Tears, My Rewards® Program Timing

Q. When does my card expire?

A. Your card will expire one year after first use or by the expiration date on the front of your card; whichever comes first. Please refer to the “Good Thru” month and year on your MTMR Card for the expiration. Your Card is valid through the last day of that month.


Q. How can I re-enroll in the program?

A. Once one year has passed from your first use of your My Tears, My Rewards® Card or your MTMR Card expires, you can re-enroll in the program.


Q. Can I use the RESTASIS® My Tears, My Rewards® Card to purchase other items ?

A. No, it is only good toward discounts on the purchase of RESTASIS® Ophthalmic Emulsion. However, if you are redeeming your prescription by mailing in your pharmacy receipts using the Independent Rebate Redemption Form, you can use the funds credited to your MTMR Card wherever RESTASIS® is sold.


Q. When will I receive my savings?

A. Savings for each respective RESTASIS® prescription refill are applied when you use your RESTASIS® MTMR Card at the pharmacy. If you are using the Independent Rebate Form (for mail-order pharmacies that do not accept the card), allow 3 to 4 weeks for your RESTASIS® MTMR Card to be credited the amount due.


Q. Why didn't I get the full amount of instant savings?

A. The instant savings are in the amount of your prescription co-pay, up to $10 for a 30-day prescription, and $90 for a 90-day prescription.* If your co-pay is less, you are only entitled to that amount of reimbursement toward your RESTASIS® Ophthalmic Emulsion prescription refill purchase.

You can increase your potential savings another $10 if you refill within 40 days of your last prescription for a 30-day prescription or within 100 days of a 90-day prescription.

Customized status updates will be provided to you in monthly e-mails to help you manage your savings and RESTASIS® refills.


Q. How is the My Tears, My Support Program different from My Tears, My Rewards®?

A. As a member of My Tears, My Support™, you'll receive the same helpful e-mails to counsel you through your treatment that the members of My Tears, My Rewards® receive. And you'll have access to the RESTASIS® Patient Support Center, where trained representatives and nurses can answer your questions about treatment and address any concerns you may have. My Tears, My Rewards® members, however, receive financial rewards and a card to use at the pharmacy.


Q. Whom can I contact if I have questions about REFRESH® Brand Lubricant Eye Drops and usage with RESTASIS®?

A. You can call the Patient Support Center where trained representatives and nurses can answer your questions about treatment and address any concerns you may have. Call the Center at 1-866-572-5931.

*According to state and federal laws, this offer cannot be extended to people whose prescriptions are paid for in part or in whole by Medicare, Medicaid, or any similar federal or state healthcare program. Patients who are ineligible for MTMR due to insurance coverage can sign up for the My Tears, My Support™ Program.

The formulary status and coverage for RESTASIS® varies greatly across insurance plans. The actual reimbursement for a member's out-of-pocket costs for RESTASIS® will vary according to refill quantity, personal healthcare insurance coverage, and following product information dosing guidelines. If a member receives reimbursement, they may still be responsible for some out-of-pocket costs. Please review the RESTASIS® My Tears, My Rewards® Program guidelines to learn about savings eligibility.

Important Safety Information
Do not use RESTASIS® Ophthalmic Emulsion if you are allergic to any of the ingredients. To help avoid eye injury and contamination, do not touch the vial tip to your eye or other surfaces. RESTASIS® should not be used while wearing contact lenses. If contact lenses are worn, they should be removed prior to the use.

The most common side effect is a temporary burning sensation. Other side effects include eye redness, discharge, watery eyes, eye pain, foreign body sensation, itching, stinging, and blurred vision.
