My Tears, My Rewards® Program Information
Q. How does the RESTASIS® MTMR Program work?
A. Each time you are ready to refill your RESTASIS® Ophthalmic Emulsion prescription, have your RESTASIS® MTMR Program Card ready and follow these instructions.
Q. What do I do if my RESTASIS® MTMR Program Card doesn't work?
A. Please call Therapy First Plus at 1-800-422-5604.
Q. How can I obtain a replacement RESTASIS® MTMR Program Card if mine is lost or stolen?
A. Please call customer service at 1-866-401-4219.
Q. What can I do if I forget to bring my RESTASIS® MTMR Program Card to the pharmacy?
A. If you have used your card at the same pharmacy for a previous prescription, they may have your ID number on file. If so, the pharmacy can fund your card by calling Therapy First Plus at 1-800-422-5604. Please provide the RESTASIS® MTMR Program Card number on the front. This will allow the pharmacy to redeem the card and pay the allowed benefit. If your pharmacy does not have your number, please call customer service at 1-866-401-4219.

Q. How can I switch from the MTMR 30-Day Program to the MTMR 90-Day Program?
A.
When you fill a 90-day prescription using your RESTASIS® MTMR Program Card, you're automatically upgraded. If you switch back to the 30-day prescription program, you will need to re-start your reward sequence.
Please note that once you end your participation in one program and begin a new program, you may need to forfeit any accumulated rewards from the previous program.
Q. After I receive my free 30-day supply of RESTASIS®, can I enroll in the program again?
A. You will automatically be re-enrolled in the program. Use the same RESTASIS® MTMR Program Card when filling your RESTASIS® prescriptions at the pharmacy.

Q. When does my card expire?
A. Your RESTASIS® MTMR Program Card expires in June 2012.
Q. When/How will I receive a replacement card?
A. Active MTMR Program members will receive replacement cards prior to the June 2012 expiration date.
Q. Can I use the RESTASIS® MTMR Program Card to purchase other items?
A. No, it is only good toward discounts on the purchase of RESTASIS® Ophthalmic Emulsion.
Q. When will I receive my savings/rewards?
A. Savings for each respective RESTASIS® prescription refill are applied when you use your RESTASIS® MTMR Program Card at the pharmacy. If you are using the Independent Rebate Form (for mail-order pharmacies that do not accept the card), allow 3 to 4 weeks for your RESTASIS® MTMR Program Card to be credited the amount due. For rewards, please allow 6 to 8 weeks for delivery.
Q. Why didn't I get the full amount of instant savings when I filled my prescription?
A. The instant savings are in the amount of your prescription co-pay up to $25 with your first prescription, and up to $15 for a 30-day prescription refill/up to $45 for a 90-day prescription refill. If your co-pay is less, you are only entitled to that amount of reimbursement toward your RESTASIS® Ophthalmic Emulsion prescription refill purchase.

Q. I have waited 3 weeks for the Web site to update since I last used my card, and my account still hasn't been credited. Whom should I contact to resolve this issue?
A. Call the RESTASIS® Patient Support Center at 1-866-572-5931 to check the status of your account.
Q. I've forgotten my Password. What do I do?
A. Click the "Forgot Password" link on the Home Page of MyTearsMyRewards.com. You will be prompted to enter the e-mail you used when you signed up for the MTMR Program. Press "Submit" to send the request and you will be sent your password within one-half hour.
Q. After using the Forgot Password link, I received a message saying I entered the wrong e-mail address or password for my online account. How can I get the correct login information or correct this problem if I think I got this message in error?
A. Call the RESTASIS® Patient Support Center at 1-866-572-5931.
Q. I don't have regular access to the Internet. Can I check the MTMR account by telephone?
A. Yes. You can call the RESTASIS® Patient Support Center at 1-866-572-5931 to check the status of your account.
Q. What will Allergan, Inc. do with the information I provided in the online survey?
A. Your information, along with the information of other program members, is used to learn more about Chronic Dry Eye sufferers and our program members. This information will also be used to continually improve the MTMR Program. Your personal information is not shared with anyone and is not disclosed to any third-party individuals or government agencies, except as may be specifically required by law.
Indication:
RESTASIS® Ophthalmic Emulsion helps increase your eyes’ natural ability to produce tears, which may be reduced by inflammation due to Chronic Dry Eye. RESTASIS® did not increase tear production in patients using anti-inflammatory eye drops or tear duct plugs.
Important Safety Information:
RESTASIS® Ophthalmic Emulsion should not be used by patients with active eye infections and has not been studied in patients with a history of herpes viral infections of the eye. RESTASIS® should not be used while wearing contact lenses. If contact lenses are worn, they should be removed prior to use.
The most common side effect is a temporary burning sensation. Other side effects include eye redness, discharge, watery eyes, eye pain, foreign body sensation, itching, stinging, and blurred vision.
